You can register with the practice online, which is our preferred method too.

Step 1:  Check you live in our catchment area to avoid disappointment

We are happy to register new NHS patients who currently live within our catchment area. Please use the interactive map below to check whether you fall within our catchment area.

Step 2:  Complete a registration form

Step 3:  Download the NHS App
  • Use the NHS App first to create your NHS Login . You need a UK mobile phone number, email address and Photo ID (Passport, UK/European Driving License or European ID card). It can help to have your NHS number at hand.
Step 4:  Await confirmation that you have been registered & request Online Access to your Records

We aim to register you as quickly as possible and within 5 working days. If you have given us a UK mobile phone number, we will send you a text message to confirm you have been registered. If you do not receive such a message, you can email us at admin.gurunanak@nhs.net for an update.

Step 5: Registration Medical – Patient NHS Health Check

To complete the registration process, you are required to have a Health Check with our Health Care Assistant or Nurse who will check your blood pressure and offer advice on healthy lifestyle.

If you are between 40 and 74 you may be invited for an NHS health check as part of the registration process even if you do not have a long term medical condition.

This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Please bring any medications that you are currently taking.

It is a 10-minute appointment and needs to be booked at the time you receive the appointment link when we are processing your registration and before an appointment can be booked with the GP.

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident you will need to complete a Temporary Resident Form. Please return the completed form to the surgery.

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours but less than three months.
