Guru Nanak and Botwell Medical Centre are part of the South Southall Primary Care Network.
Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.
To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas in groups of practices known as primary care networks (PCNs).

South Southall Primary Care Network is an NHS Collaboration between 8 GP Practices in Southall:
Featherstone Road Health Centre, Belmont Medical Centre, Guru Nanak Medical Centre, Hammond Road Surgery, Southall Medical Centre, Sunrise Medical Centre, The Welcome Practice and Waterside Medical Centre. We have excellent relationships across the 8 practices and are working collaboratively to provide extended services to address any inequalities and to provide great care to our patients and 1 Director Dr Ajan Sivanesan.
Across the PCN, the additional service provide includes:
- One Mental Health Nurse, who supports patients with mental health concerns and works closely with GPs and community services.
- Two First Contact Physiotherapist, who assesses and manages musculoskeletal problems such as back, neck, and joint pain, often without the need to see a GP first.
- Two Social Prescribers, who support patients with non-medical needs such as social isolation, housing, financial concerns, and access to community services.
- Seven Clinical Pharmacists, who help with medication reviews, long-term condition management, prescription queries, and medication safety.
In addition to the ARRS roles (Additional Roles), each practice has GP Assistants, who support clinical work and help improve efficiency and patient flow.
We also have:
- One Digital Transformation Lead, who supports digital systems, access, and patient-facing digital services.
- Two Paramedics, who work within primary care to assess urgent and same-day clinical needs.
The aim of this team structure is to ensure patients are seen by the right professional at the right time, improving access while maintaining high-quality, safe care.
